Downloading and using Adobe Acrobat Reader

All of the applications or enrollment forms available for download from our website are in the Adobe Acrobat (PDF) format. This format allows you to view and print an enrollment form in the same format it appears in print. The PDF format is both device and resolution-independent, so the printed version of a PDF will be similar regardless of the device used to print them or the resolution and fonts used by the device. All you need is Adobe's free Acrobat Reader installed on your computer in order to view and print these forms.
