Thanks to very affordable group rates now available to physicians, it makes good sense to add an Accidental Death and Dismemberment Insurance Plan to your health protection program. In the event of a covered loss as a result of a covered accident (with the result of death or dismemberment as described in the plan), the AMA-Sponsored Premier Accident Plan will pay up to $1,500,000* directly to you or your family. Benefits are paid in addition to any life, health or disability insurance benefits received, and may be used for any purpose. All covered family members, including your spouse or domestic partner and children, are covered anywhere in the world, 24/7.

Benefits are payable for insured losses which occur within 365 days after the covered accident. This plan includes provisions for losses due to exposure to the elements and for an insured's disappearance following a covered accident.

The benefit amount you select is the maximum benefit payable for losses resulting from any one covered accident (excluding any extra protection benefits). Benefits for insured family members are at a fixed percentage of the benefit amount you select.

Please Note: If you insure both your spouse and dependent children, your spouse will be insured for 40% of the benefit amount you select and dependent children will be insured for 10% of your benefit. When you and your spouse are the only insureds, your spouse's benefit increases to 50% of the benefit your select. When you and your child(ren) are the only insureds, the benefit for each child increases to 15% of your benefit.

Provides coverage for accidental loss of life, limbs or sight
This is an accident protection plan that recognizes the financial concerns your family will have if you die or lose use of a limb as the result of a covered accident. As such, it pays the full benefit for the loss of life, any two limbs or sight in both eyes or any combination of limbs and eyes; 50% of the full benefit is payable for the loss of a single limb or eye.

A Benefit Designed for Physicians: Loss of Use of Limbs
This Plan can pay the full benefit amount you select - up to $1,000,000 - if a covered accident results in the permanent loss of the use of any two limbs.

Choice of Benefit Levels up to $1 million
You can choose the coverage that is right for you: $100,000 $200,000, $300,000, $400,000, $500,000, $750,000 or $1,000,000. Coverage for your eligible dependents is a percentage of the benefit amount you select.

Note: Benefits reduce by 50% at age 70.

Covered Losses
The benefit amount you select is the maximum benefit (excluding any applicable extra protection benefits) payable for losses resulting from any one covered accident.

Benefits are payable for losses that occur within 365 days after a covered accident; due to exposure to the elements and for an insured's disappearance following a covered accident. This plan can pay up to the full benefit you select from an accident that occurs while you perform professional services as part of your regular job obligations in a helicopter or other aircraft utilized as an air ambulance.

Benefits Paid for Covered Losses

  • Up to $1,000,000 (the full benefit amount) for loss of: life, both hands, both feet, one hand and one foot, or irrecoverable loss of their use; sight in both eyes.
  • Up to $500,000 (50% of the full benefit amount) for loss of: one hand or one foot, or irrecoverable loss of its use; sight in one eye.
  • Up to $250,000 (25% of the full benefit amount) for loss of: thumb and index finger, on one hand.

Additional Benefits Paid if Loss of Life benefit is payable

  • Up to $500,000 (50% of the benefit selected) for loss of life resulting from injuries as a fare-paying passenger on any form of public conveyance.
  • $50,000 or 50% of your full benefit amount, (whichever is less) for death within 90 days of an automobile accident where seat belts were properly worn.
  • Up to $5,000 for covered expenses incurred to return the insured's body to the United States, if death is due to a covered accident. Covered expenses include, but are not limited to, expenses for embalming, cremation, coffin and transportation.
  • Up to $10,000 annually (for up to 4 years) for education expenses of insured spouse, domestic partner or dependent children who are entrolled as a full-time student in a college or trade school within one year of the insured member's accidental death.

Eligibility

You are guaranteed acceptance in this plan if you are under age 70 and actively engaged as a full-time physician, or retired for reasons other than illness or injury. Your spouse or domestic partner, under age 70, and all your unmarried, dependent children through age 18 (through age 26 if unmarried and full-time students) are also guaranteed acceptance.
Individuals covered under the American Medical Association (AMA)-sponsored AD&D Plan for the AMA Alliance are not eligible for this insurance protection.

Effective Date

Your coverage is effective upon receipt of your application, provided the premium is paid when due.

Renewability and Termination

You can renew your coverage up to the premium renewal date immediately following your 80th birthday - as long as premiums are paid on time, the AMA does not sponsor a similar plan and the group policy remains in force.

Exclusions

No benefits are paid for losses that result from or are caused directly, indirectly, wholly or partly by: war or act of war, declared or undeclared; suicide or intentionally self-inflicted injury while sane or insane (for Missouri residents, suicide or self-inflicted injury while sane); mental infirmity; bacterial infections (except infections occurring simultaneously with and due to an accidental injury), or any kind of sickness or disease; injuries sustained while acting as a pilot or crew member on any aircraft, or as an aircraft passenger except when flying in: 1) licensed passenger aircraft operated by a duly licensed pilot, or 2) transport aircraft operated by the United States Military Air Command or similar service of Canada or Great Britain.
