Level Term Life Insurance is recommended for providing solid, level protection for a time frame that meets your short or long term goals - all for a cost that can comfortably fit your budget.

The AMA-sponsored Level Term Life plan allows you to customize your protection by applying for a coverage amount in $1,000 increments from $100,000 up to a maximum of $2 million.

And, unlike other life plans that may go up in cost over time, this plan maintains the coverage amount you select at a fixed rate for your choice of 10, 15, 20 or 30 years. The plan also includes a renewability option should you wish to continue your family's protection beyond the original term.

You can add coverage for your spouse or domestic partner at the same affordable group rates, and also add each eligible child for a flat rate of $60 a year. Another feature of this plan allows spouses or domestic partners to apply for coverage even if you choose not to apply.

Features & Benefits

Benefits up to $2,000,000
This plan allows you to select the amount of term life insurance coverage that best fits the financial needs of your family. As an eligible physician, you and your eligible spouse or domestic partner can each apply for coverage from $100,000 up to $2,000,000.

Because this is level term coverage, the benefit amount you secure remains level for the entire term of your coverage.

Level term period - rates guaranteed for 10, 15, 20 or 30 years
You select the level period that makes sense for you and your family's personal protection needs - 10 years, 15 years, 20 years or 30 years. The rate you are approved for is the rate you keep for the entire level period. There are no annual increases during your chosen level period, guaranteed.

How Rates are Determined
This plan's rates are based on various factors including your lifestyle, health history, and the company's underwriting guidelines. Four (4) rate classes are offered:

  • Preferred Plus rates are this plan's lowest rates. They're designed for physicians who have not used tobacco in any form during the past 12 months and whose lifestyle and health history meet the Preferred Plus underwriting guidelines.
  • Preferred rates are for physicians who have not used tobacco in any form during the past 12 months and whose lifestyle and health history meet the company's Preferred underwriting guidelines.
  • Standard rates are for individuals who have not used tobacco in the past 12 months.
  • Tobacco rates are for individuals who have used tobacco in the past 12 months. Rates for tobacco users are very competitive.

Up to $500,000 in "Living Benefits"
You can receive up to 60% of your basic benefit amount - up to a $500,000 maximum - to use any way you wish if you are diagnosed by your attending physician as having no longer than 12 months to live. In addition, your premium payments will be waived during the 12-month period following payment of this benefit. This added feature is provided at no additional cost to you. "Living Benefit" payments are deducted from your total death benefit. Please note: "Living Benefits" may be taxable. Please consult with your personal tax advisor for details.

Optional Features to Tailor Your Coverage

Waiver of Premium
If you're actively working as a physician full-time and you become totally disabled after the effective date of coverage, before age 60, and the disability lasts for at least six consecutive months, your premium payments will be waived up to age 65. To take advantage of this option, you must apply and be approved for coverage before age 56. Coverage for this option terminates at age 60.

Accidental Death and Dismemberment Coverage
This optional coverage provides additional insurance protection equal to your basic life insurance benefit (up to a $500,000 maximum) if death is caused by a covered accident. In addition, benefits are payable for loss of limbs or loss of use of limbs, or loss of sight due to a covered accident. Coverage for this option terminates at age 75.

$25,000 Child Coverage
For an additional $60 a year, you or your spouse (but not both) can cover each of your eligible dependent children with $25,000 of life insurance protection. Eligible children include all unmarried dependent children from 6 months to 19 years (through 26 if unmarried, dependent and full-time students).

Note: In Montana, females pay male rates.

Eligibility

If you are an actively engaged full-time physician or full-time medical student under age 61, you and your spouse or domestic partner under age 61, are each eligible to apply for the American Medical Association (AMA)-sponsored Level Term Life Insurance plan.

All applications are subject to medical underwriting and approval. Any required medical exam will be conducted at your convenience and at no cost to you. AMA membership is not required.

Effective Date

Your coverage will become effective on the 1st or 15th of the month coinciding with or next following the date your application is approved by United States Life Insurance Company in the City of New York. Coverage will be in force from the effective date, provided the premium is paid when due. Physicians must be actively at work on the date the insurance is to take effect. If not, the insurance will take effect on the day they return to work. Medical students, spouses or domestic partners and children must not be hospitalized on the date the insurance is to take effect. If they are, the insurance will take effect on the day after they are discharged.

Renewability and Termination

You may re-enter the plan every 10, 15, 20 or 30 years (based on the level period you select) at the rate then in effect for your attained age and risk class, as long as you are under age 61 and can provide satisfactory evidence of insurability.
After age 61, or if you cannot provide satisfactory evidence of insurability at the end of a 10, 15, 20 or 30-year period, your premiums will increase based on age on a yearly basis.

You can continue to renew your coverage through age 94. Coverage will end earlier if the group policy ends, insurance for your class ends, or you fail to pay the premium when due.

Exclusions

Life Insurance: suicide, whether sane or insane, during the first two years of coverage is limited to a refund of total premiums paid (except for Missouri residents for whom suicide while insane is no defense to payment of life insurance benefits).
Accidental Death and Dismemberment Insurance: flight in aircraft for training, testing or experimental purposes; ptomaine, bacterial infections (except if such infections result from the accidental ingestion of a poisonous food substance), or any other kind of disease; war or any act of war declared or not; mental or physical infirmity; or suicide or intentionally self-inflicted injury while sane or insane are not covered (except for Missouri residents for whom suicide while insane is no defense to payment of accident benefits).
