Preferred Term Life Insurance
If you need a substantial amount of coverage, either to protect your young and growing family, or to protect the nest egg you've worked so hard to build over the years, we recommend the AMA-sponsored Preferred Term Life Insurance Plan. It offers up to $3,000,000 of solid protection that is renewable up to age 75. The rate is lowest at younger ages, and adjusts every 5 years to parallel your increasing age and higher income.
Your spouse or domestic partner is also eligible if you choose to apply for coverage, and they are eligible for their own protection at the same affordable group rates, even if you choose to forego coverage. They may also add children to their plan.

Features & Benefits
Benefits up to $3,000,000
As an eligible physician, you and your eligible spouse or domestic partner can each apply for the amount of coverage that is most appropriate for the financial needs of you and your family - from $25,000 up to $3,000,000.
Benefit Reduction
Once insured, your benefit stays the same to age 70, regardless of changes in your health. Your benefit amount and corresponding premium both reduce by 50% on the premium anniversary date following your 70th birthday.
Unique "Age at Entry" Rate Feature
For the first five years your rate is based on your age at issue. Rates may only increase during that time if they are raised for the entire group based on claims experience. After the initial five-year period, your rate is guaranteed to be based on your attained age at each subsequent five-year interval and will not increase due to age during each five-year period unless rates for the entire group increase. This is a unique feature of the AMA-sponsored Preferred Term Life Plan designed much like a level term product to give you the most attractive rate possible.
Accelerated Benefit Feature
Should you be diagnosed as having less than 12 months to live (24 months in some states), the plan may advance you up to 60% of your basic life insurance amount, subject to a maximum of $500,000. You can use the money to pay medical bills, outstanding debts, or for anything you choose.
Your premium payments will be waived for up to 12 months following payment of this benefit. Future premiums will not otherwise be affected by payment of the benefit and will be required to be paid. Your benefit advance is payable only once and will be deducted from your total life insurance coverage. Note: Accelerated Benefits may be taxable. Please be sure to consult your tax advisor.
Optional Features to Tailor Your Coverage
Waiver of Premium
Allows your life insurance premiums to be waived during a period of covered Total Disability beginning before age 60 (while insurance is in force) and lasting at least 9 consecutive months. Your life insurance will remain in force until your disability ends or you turn 65, whichever occurs first, without further payment of premium. This protection is available to all practicing full-time physicians and their actively employed spouses age 54 or younger.
Accidental Death and Dismemberment Coverage
This optional coverage provides additional insurance protection equal to your basic life insurance benefit (from $25,000 up to a $500,000 maximum) if death is caused by a covered accident. In addition, benefits are payable for the total and irrecoverable loss of both hands or feet or loss of use of both hands or feet, or loss of sight due to a covered accident. (Loss must occur within 365 days of the accident). Coverage for this option terminates at age 70.
Dependent Child Coverage
Lets you or your spouse (not both) insure all your eligible unmarried, dependent children ages 6 months through 19 years (continued up to age 27 if unmarried, dependent and a full-time student) with $25,000 of term life insurance and $25,000 of accidental death and dismemberment insurance each. Once this option is in place, each new child is automatically insured beginning at age 6 months. Each child's coverage ends when the child is no longer eligible or when the insured parent's coverage ends (except through death).
How Rates are Determined
- This plan's rates are based on various factors including your lifestyle, health history, and the company's underwriting guidelines. Four (4) rate classes are offered:
- Preferred Plus rates are this plan's lowest rates. They're designed for physicians who have not used tobacco in any form during the past 12 months and whose lifestyle and health history meet the Preferred Plus underwriting guidelines.
- Preferred rates are for physicians who have not used tobacco in any form during the past 12 months and whose lifestyle and health history meet the company's Preferred underwriting guidelines.
- Standard rates are for individuals who have not used tobacco in the past 12 months.
- Tobacco rates are for individuals who have used tobacco in the past 12 months. Rates for tobacco users are very competitive.
Eligibility
If you are an actively engaged full-time physician or a full-time medical student under age 61, you and your spouse or domestic partner under age 61 are each eligible to apply for the AMA-sponsored Preferred Term Life Plan. Your spouse or domestic partner is eligible to apply even if you choose not to.
All applications are subject to medical underwriting and approval. Any required medical exam will be conducted at your convenience and at no cost to you. AMA membership is not required.
Effective Date
Your coverage will be effective on the 1st or 15th of the month coinciding with or next following approval by The United States Life Insurance Company in the City of New York. Coverage will be in force from the effective date, provided you are actively engaged full time in the practice of medicine and the premium is paid when due.
To make this insurance available at the lowest possible rate, financial underwriting, a medical examination and/or other additional health information will be required in most instances when applying for more than $100,000. The effective date of coverage with respect to your spouse or domestic partner or eligible, dependent child(ren) confined to a hospital on the date the insurance would otherwise become effective will be deferred until the day after the spouse or domestic partner or dependent child(ren) is discharged.
Renewability and Termination
Coverage for you and your spouse or domestic partner is renewable to age 75 at the premium rates and benefit levels current for your age and class of risk current at that time. Coverage will terminate if your premiums are not paid when due or the group policy is terminated. Your covered spouse and dependents may keep their insurance even after your death.
Exclusions
Life Insurance: suicide, whether sane or insane, during the first two years of coverage is limited to a refund of total premiums paid (except for Missouri resident for whom suicide while insane is no defense to payment of life insurance benefits).
Accidental Death and Dismemberment Insurance: flight in aircraft for training, testing or experimental purposes; ptomaines, unless accidentally ingested; bacterial infections (except pyogenic infections which occur simultaneously with and through an accidental cut or wound), or any other kind of disease; war or any act of war declared or not; mental or physical infirmity; or suicide or intentionally self-inflicted injury while sane or insane are not covered (except for Missouri residents for whom suicide while insane is no defense to payment of accident benefits).
